Ingest a FHIR batch Bundle

A FHIR R4 `batch` Bundle only (each entry processed independently). A `transaction` Bundle is refused with `422 transaction_not_supported`; other Bundle types go to `$ingest`. `Accept: application/fhir+json` answers a `batch-response` Bundle.

1–255 characters of [A-Za-z0-9._:-] (a UUID v4 is recommended); never PHI. Stored 24 hours per app.

Ingest another Bundle type ($ingest) POST

`$ingest`: Bundle types FHIR does not allow at the base (`collection`, `searchset`, `history`), including Subscriptions Backport notifications. Handshakes and heartbeats produce no events.
